Starting with 5 October 2021, the epidemiological form must be completed at least 3 days before entry into the Republic of Moldova by persons who:
- Do not hold a COVID-19 vaccination certificate;
- Do not have a negative result of an RT-PCR test valid for 72 hours or a rapid antigen test valid for 48 hours;
- Do not have a confirmatory medical conclusion in the case of persons who have been sick with COVID-19 between the 15th day and the 180th day (6 months);
- Do not have a confirmatory certificate of the presence of anti-COVID-19 antibodies, valid for 90 days from the date of the investigation.
If passengers do not have one of the above confirmatives, they must comply with the self-isolation regime for 14 days.
Persons in self-isolation regime (quarantine), regardless of the area from which they have arrived, may interrupt this regime after the 7th day if they perform a COVID-19 PCR test and the result is negative.
